About AxcessPointe Health System
Our integration with AxcessPointe Health System brings together two organizations with a shared commitment to improving community health and addressing barriers to care. By joining the APHS network, Opportunity House will gain access to additional resources and community partnerships while continuing its mission of supporting individuals and families in need.
Formerly known as Cabarrus Rowan Community Health Centers, AxcessPointe Health System is a community-based healthcare organization serving nearly 20,000 patients across Cabarrus, Rowan, Mecklenburg, and Forsyth counties.
With a network of 23 service sites, APHS provides comprehensive, evidence-based healthcare services, including primary medical care, dental care, behavioral health, pharmacy, urgent care, imaging, school-based care, and mobile health services.
APHS is committed to delivering high-quality, affordable, patient-centered care regardless of a patient’s ability to pay.
Moving Forward Together
Through this partnership, Opportunity House will gain access to AxcessPointe's resources and community partnerships while continuing its mission. Together, both organizations will strengthen connections between healthcare and social services to provide more coordinated support for individuals and families experiencing homelessness.
By combining resources and strengthening partnerships, AxcessPointe and Opportunity House aim to improve access to healthcare and essential support services throughout the community.
